STATE AGENCY/ INSTITUTION SEMIANNUAL REPORT ON PUBLICATIONS

Your Louisiana agency/ institution should submit the required number of copies of your public documents (listed in the accompanying email form or letter) to the Louisiana State Documents Depository Program. Please do not send extra copies.

Please list any new or revised publication that has not or can not be submitted to the

Depository Program and give the reason why, e.g. no paper format, no copies left, too few printed, etc. List any titles that have ceased, with date of last issue/edition.

If you have not told us about or submitted a title or issue/edition that is only online, please list it with the publication date and URL.
